We realize that patients with an abnormal colorectal screening or a diagnosis of colorectal cancer will be faced with the many challenges of having to make decisions regarding treatment.
Your Care Coordinator is here to assist and walk with you along the way, from scheduling appointments for tests, procedures and surgery, to putting you in touch with the latest clinical trials and physician experts.
This service is available free of charge and is designed to help you through the process of diagnosis, treatment and recovery.
Your Coordinator will provide the following services and support tailored to your individual needs:
· Rapid scheduling of appointments, tests and procedures
· Assistance with coordination of providers, tests and procedures
· Information about symptom management
· Ongoing disease information, education and support services
Your Colorectal Coordinator will help you navigate the full spectrum of services, from accessing the latest medical research, to psychological and nutritional counseling. Whether its coordinating appointments, assisting with insurance issues, or just a heart-to-heart talk about day-to-day issues affecting your treatment, your care coordinator is your advocate for quality care.
